Partnerships - Support at Home Program

TransCare has several partnerships with Support at Home Program providers that operate within our region. This means that our Commonwealth Home Support Program clients can move to another provider, if they are awarded a Support at Home package, and continue to recieve their support services through TransCare. These partnerships provide continuity of care for our clients and they make the transition to a higher needs program less complicated.

Brokerage Services

TransCare provides brokerage services to aged care and disability support providers to deliver quality and excellent care to their clients. We never approach or solicit clients to change providers.

Private Services

TransCare can provide private home care services to people who do not qualify for government funded services. Private services can also be provided to people waiting for a home care package to be allocated or perhaps when their government funded program doesn’t provide as much support as needed.
