Transport Services

CHSP Direct Transport - Provision of transport services to assist CHSP clients to remain actively connected with their local community. Transport services aim to assist clients to continue with their usual activities, such as attending community groups or medical appointments, enabling them to keep active and socially engaged.

 

NSW Community Transport Program (CTP) - The key objectives of the CTP services are to:

  • Provide services to CTP eligible customers who have no or limited access to private or public transport due to their location, when they need to travel, or their financial resources, or physical and cognitive capacities;
  • Assist CTP eligible customers to take part in activities that promote social inclusion and to obtain goods and services required for daily living such as groceries and clothing, medical services and other essential services; and
  • Provide CTP eligible customers connections to public transport.

 

Community Transport Program (CTP) Eligible Customer - Means a person who:

  • Is under 65 years of age;
  • Requires a transport service for a medical, social, recreational or shopping purpose;
  • Is not eligible to receive transport services for that same purpose, under another government program such as the CHSP, Home Care Packages, Support at Home, Residential Care, or NDIS or any equivalent or replacement programs; and
  • Has limited or no access to private or public transport due to their location, when they need to travel, or their financial resources, physical or cognitive capacities.

 

NSW Health Non-Government Organisation Grant Program - NSW Health supports the provision of non-emergency health related transport programs that enhance access to health care by catering for the travel needs of people who are transport disadvantaged.

 

Hunter New England Local Health District - The Transport for Health Unit provides funding for health related transport.
